East Renfrewshire service praised for its health, social care integration, and support provided to residents.

About fifty professionals work as a team. They are part of the local Health and Social Care Partnership. They give support to people in East Renfrewshire to help them stay independent at home and after they leave the hospital.
Referrals to the service went up a lot. Over the last six years, they increased by 80%. That’s a significant jump in people needing help.
The service has many different specialists. These include physios, occupational therapists, dieticians, nurses, and speech therapists. They all work together for good care.
The team adapted to serve older people. Their goal is to help them stay independent, whether at home or in care homes. East Renfrewshire has thirteen care homes.
The service recently created a new role, the Advanced Frailty Practitioner. Every care home now has a link physio and a link occupational therapist.
